Volume 3 contains information from Volumes 1 and 2, the 1992 Supplement and the 1995, 1996, 1997 and 1998 Updates as well as new information submitted before December 31, 2004.

Volume III is sold out!

We don't know if there will be enough interest to order more books.  There is a minimum order.  We do not know what the cost will be.
